Digital photographs of personal and address ID for the client, ID sheet and ‘True Likeness’ form completed
Thorough investigation of possible LEI sources, LEI checklist and clear copies of any insurance documentation provided
Explanation of the funding process and completion of a CFA checklist signed by the client and agent
Explanations of, and collection of all relevant signed documentation that is required by the fee earner
Collection of any relevant documentation that the client may have in relation to the claim
Verification of identity documentation, with photographic evidence

The Fee: £85.00 (plus vat)

2. Full Statement

Production of a detailed Liability and/or Quantum soft copy and printed statement, which is verified and signed by the client, and emailed to the solicitor for review, if required
Witness details will be included; where possible witnesses to a defect, to the accident and to the aftermath of the accident
Any persons or organisations mentioned within the statement will have full contact details included
This service can include the full ‘CFA Sign-Up’ as outlined above

The Fee: £248.00 (plus vat).

3. Employer Liability Statement

In addition to the above ‘Full Statement’, a sketch of the accident scene and images of similar machinery or equipment, if the accident happened in a working environment. We will attempt to obtain Operator Manuals and witness evidence to general standards of working practice. Evidence of previous accidents.

4. Witness statement

Production of a detailed soft copy and a printed statement, which is verified and signed by the witness, and emailed to the solicitor for review, if required
Witness ID can be supplied but, if the witness refuses, the agent will not pressurise the witness

5. Questionnaire

Production of a legible, hand written detailed Questionnaire, which is signed by the client at the time of the meeting
Any persons or organisations mentioned within the statement will have full contact details included
Copies of the various questionnaire templates can be provided for review
This service can include the full ‘CFA Sign-Up’ as outlined above

6. Locus Report (Slip & Trip)

The client will be in attendance at the accident site whenever possible. If the client cannot attend the site due to injuries etc. then a photograph will be shown to the client and verification sought
Several photographs of the defect, with clear measurements, and the surrounding area will be provided; direction of travel will be shown One photograph will have the client pointing at the specific defect
A map of the area and at least one ‘Witness to the Defect’ form will be provided. If the agent feels that the defect is not actionable, the agent will contact SYN4 before undertaking any further work. The fee earner will be emailed the photographs and asked for direction before the instruction is completed
Detailed diagrams with measurements, will be provided, if appropriate and relevant

7. Locus Report (RTC)

Production of a detailed plan of the accident site, with a detailed diagram of the accident details, including measurements and direction of travel for all vehicles involved, along with photographs taken at a similar time/day of the week that the accident occurred
Technical details of the accident site, including road signs, road works, speed limit, road markings, possible hazards etc, along with photographs
Any changes that have occurred to the accident site
Where possible, investigate whether any previous accidents have taken place at the accident site

The Fee: £179.00 (plus VAT).

8. Lost Contact – Case Recovery

If contact with the client cannot be made by phone, the agent will ‘cold call’ at the address supplied. If there is no one at the address provided, the agent will leave a letter requesting contact from the client and speak with neighbours, if possible, to establish if the client still lives at the address
If the Agent meets with the client they will confirm that the client wishes to proceed, collect any relevant documents at that time or arrange another convenient time to meet with the client. Current or updated contact details will be obtained and returned to the fee earner
Following the meeting, any relevant signed documentation, photographs etc will be returned to the fee earner
If the ‘recovery’ is unsuccessful, a photograph of the outside of the property will be supplied, to confirm that the Agent has visited the address
